At almost 20 years old, my areolas nearly cover my entire breast, extending more than halfway up and are puffy. This results in a mound-like appearance on top of my actual breast. When wearing bras, my areola and nipple tend to fold down, giving me an odd shape due to their larger volume compared to my breast. It seems as though my breast and areola are two separate entities since there is a fold in the middle. When I lie sideways, my areola and nipple remain in place while the underlying breast folds to the side.
Your concerns can be addressed. Nipple-areolar reduction can help correct the puffiness and large size you described. If you have tubular breasts, this can also be addressed during the same procedure. I recommend consulting with a board-certified plastic surgeon in your area.
