Almost four weeks ago, I underwent a breast reduction and the wound at the T junction appears quite alarming. When the steristrips were removed, there were blisters underneath and some stitches that were coming out had to be removed. I was advised to apply ointment and cover it, but to me, it seems to be worsening. The black spots in the wound are not scabs, so could this be necrotic? How often should I be changing my dressings?
Hello there. Plastic surgeons refer to the area where three suture lines come together as the "angle of sorrows." It is a known area that can experience breakdown in some cases, like yours. There is no need to worry, though. While the breakdown appears to be moderate in size, it appears clean, and the rest of your wound is healing well, indicating no active infection. Follow the instructions of your dressing nurses and apply antibiotic ointment daily with non-occlusive dressing. Change the dressing every day or two, depending on its condition. Over the next few weeks, the wound will begin to contract and heal. This approach minimizes scarring and reduces the likelihood of needing additional surgery. The body will naturally remove any infected material, and you can expect full healing within three months. The only cause for concern would be if you experience long-term drainage (8-12 weeks), increasing redness, pain, swelling, or foul discharge. If you notice these worrying signs, please consult your general practitioner or surgeon immediately. Best wishes.
It seems that you are experiencing wound healing issues at the T-junction after breast reduction surgery. This is not uncommon, and while I do not see any necrotic tissue in the photos, it is difficult to assess the black area. It is important to change your dressings twice a day and follow your surgeon's instructions.
Necrosis at the T-junction is a common occurrence and doesn't appear to be infected. Follow your surgeon's prescribed local wound care for optimal healing.
