I am 7 months post-operation from a breast augmentation with lift and have developed acute capsular contracture in both breasts. My implants (450cc, SRF) were placed under the muscle. My surgeon plans to perform a capsulotomy and replace them with smaller 365cc implants. Would you advise against the exchange?

Capsular contracture can occur for various reasons, including bacterial contamination during implant placement, the use of oversized implants, genetic factors, and more. On average, the rate of capsular contracture is approximately 1% per year. To minimize the risk of developing capsular contracture, I recommend specific measures such as using an inframammary incision, placing the implant beneath the muscle, employing a no-touch technique with an implant funnel, using a gentle and aseptic surgical technique, and performing an aseptic pocket wash. If capsular contracture does occur, the best approach to reduce the risk of recurrence is to remove and replace the implant in a different position.
