My surgeon informed me that due to the large size of my breasts, a free nipple graft was the only option for my breast reduction. Is it normal for a nipple graft to cause the nipple to fall off? What are some reasons why a nipple might fall off? My breasts were a 44I, and they are noticeably different in size. Are the results I am seeing 8 weeks after the reduction considered normal?
Thank you for sharing your pictures. Unfortunately, your breasts and nipple/areolar complexes appear normal for a free nipple graft. In my practice, I no longer perform free nipple grafts due to the cosmetic limitations, such as variable pigmentation return and loss of nipple projection. Instead, I prefer techniques that preserve the nipple or reconstruct it during the reduction procedure to achieve better cosmetic outcomes.
After undergoing free nipple grafting, it is possible for the skin graft to experience discoloration, dryness, infection, or even loss. Once the area has fully healed, if pigment changes persist, tattooing can be considered to restore the appearance. Additional procedures can also be performed to reconstruct the nipple/areola complex. However, it's important to note that nipples do not regrow naturally.
Superficial sloughing of the nipples is common after surgery, but the underlying tissue usually survives, and pigmentation can often return within the first 6-12 months. Tattooing may be an option if pigmentation does not fully return.
