At one year post-op, your healing process is essentially complete, and any soreness or little knots" are unlikely to improve or resolve. This is because areas of dead fat have transformed into scar tissue, not due to a lack of massage therapy. Not all transplanted fat survives, and any dead fat cells or globules turn into scar tissue or areas of fat necrosis, which can feel firm or like "little knots." While this is not harmful, it can be uncomfortable and visually irregular. (On a side note, I do not endorse fat grafting for breast augmentation due to the potential confusion it may cause in diagnosing breast cancer on mammography. Although radiologists can usually differentiate between dead fat calcifications caused by non-surviving fat grafts and those caused by cancer, I prioritize the safety of my patients by not performing fat grafting on their breasts.) Fortunately, this concern does not apply to the buttocks, as breast cancer does not develop in that area. Massage therapy would not have increased fat graft survival, and it is unlikely to provide benefit now that healing is complete. Rest assured that soreness is not a cause for concern unless accompanied by redness, swelling, or purulent drainage, which may indicate infection. Best wishes! "
