If saline implants rupture, they will deflate, and the saline will be naturally absorbed by the body. In most cases, the remaining implant does not pose a medical problem. On the other hand, if silicone gel implants rupture, the gel may remain within the breast capsule formed by the body and may not cause issues. However, if the silicone gel escapes into the surrounding breast or lymphatic tissues, it can potentially lead to complications such as infections or lymphedema. Therefore, if a silicone gel implant is confirmed to be ruptured, it should be removed and/or replaced. It is worth noting that new form-stable silicone implants, known as Gummy Bear Implants (Silimed Implants - Sientra), are available, and they do not leak even if punctured or cut, reducing the risks associated with silicone implant rupture.
Regarding saline implants, if a rupture occurs, the saline solution is harmlessly absorbed by the body since it is the same saline used in intravenous (IV) fluids during surgery. As a result, the implant will deflate, but it won't pose any harm until you have it removed or replaced. Newer silicone gel implants, known as cohesive gel implants, are highly soft and cohesive. They have a sticky consistency and can maintain their shape even if cut. This makes it easier to remove a ruptured cohesive gel implant intact. However, it can be more challenging to detect rupture through conventional methods, and an MRI may be necessary to confirm the rupture. Even with a ruptured implant, you may not experience any problems for a long period. The main concern would be the potential development of capsular contracture, which is the formation of thick scar tissue around the broken implant, leading to firmness or distortion of the breast shape. In such cases, surgical intervention may be required to remove the scar tissue and replace the ruptured implant.
Ruptured gel implants should be removed to prevent the formation of a siliconoma. Saline implants should also be addressed if ruptured.
